|Voter Registration Application||Application to register to vote. Also used to update name, address, or signature.|
|Request to receive a ballot in the mail. Can be used to request a ballot for a specific election, or to sign up for the absentee list and receive all ballots in the mail. You must be registered to vote to use this form.|
|Replacement Ballot Request Form||For voters who were issued a mail/absentee ballot and need to request a new one. This form can only be used if the original ballot has not yet been returned.|
|Ballot Collection Registry Form||Required for someone dropping off a ballot for a voter. You must provide your information, as well as the information for the voter whose ballot you are dropping off. Note that you may only drop off a ballot for another voter if you are related to them as a: caregiver, family member, household member, or acquaintance. You are limited to dropping off six ballots for voters (not including your own) per election.|
|Application for Designation of Agent||Voters with a disability use this form to appoint someone to assist them with voter registration, ballot requests, and voting. Authorizes voter's agent to sign on behalf of the voter. Form must be completed in front of two witnesses - see form for more details.|
|Request for Voter File or Absentee List||Request for list of registered voters or list of absentee voters. List may be purchased for noncommercial use only.|
|Land Owner Request for Fire District Election Ballot||Owners of property in a fire district holding an election who are not already registered to vote in that district may use this form to request a ballot. Proof of payment of taxes on the land in the fire district must be submitted with this form.|
