NEW UPDATE: All enrollment forms are due to the Extension Office with the $10.00 membership dues by December 31st. Enrollments forms received after December 31st will require$50.00 membership dues.
No enrollment forms will be accepted after February 1st.
4-H Enrollment Form for NEW MEMBERS (club leader signature required with $10 yearly fee)
New Volunteer Enrollment Packet ($30 background fee plus $10 yearly dues)
Re-enroll forms for Certified Volunteers ($10 yearly dues)
4-H Project Plan (for the Self Determined project and Independent Study options due January 15)
Beef Tagging Form - January 3, 2018
Swine Tagging Form - due April 2018
Sheep Tagging Form - Due May 2018
Goat Tagging Form - Due May 2018
McDonald's - for new members who need a place to raise an animal
Animal Project Permission Form (required for all animals - big and small except horse)
Dog Registration Form - Must be in by December 16, 2017
Volunteer Appreciation Awards form - Due April 1
Scholarships Application - due April 1
Ambassador application - due May 1
Grievance Policy - due within one hour of infraction
